MULTICENTER EVALUATION OF TIME, OPERATIONAL AND ECONOMIC EFFICIENCIES OF A NEW PRE-LOADED IOL DELIVERY SYSTEM VS MANUAL IOL DELIVERY

Purpose:
To compare the cataract surgery intra-operative lens delivery time, device preparation and total intraoperative surgical time when using a new pre-loaded IOL delivery system in comparison with manually loaded IOL delivery system for use in routine, uneventful cataract surgeries. This study collects intra-operative time data for staff involved in surgery when using different IOL delivery systems (Monarch, Ultrasert, iTec and iSert).
Methods:
The study included 240 surgeries, 131 in France and 109 in Spain, in a prospective observational, time and motion evaluation. Delivery device preparation was recorded by an external video camera, intra-operative procedure was recorded by the microscope camera. Two independent graders assessed the videos and recorded time of the lens delivery phases. Surgery labour utilization was also examined.
Results:
Overall, mean lens delivery time was 14.0 sec and 12.2 sec for Ultrasert pre-loaded IOL delivery system and Monarch manual delivery system respectively (p 0.178). However in both countries, mean device preparation time was significantly decreased with Ultrasert device compared to Monarch (France, 48.4 sec for Ultrasert vs 62.3 sec for Monarch (p 0.004). Spain, 27.6 sec for UltraSert vs 69.5 sec for Monarch (p<0.001)). In Spain total intraoperative time was significantly decreased with Ultrasert device compared to Monarch (41.4 sec vs 83.0 sec).
Conclusions:
A significant reduction was observed with regards device preparation and intraoperative time when using Ultrasert delivery system compared to Monarch however no difference for lens delivery time between both devices was observed.
